E-Motion Physical Therapy & Wellness ("E-Motion," "we," "us," or "our") is the name under which two practices operate: E-Motion Physical Therapy PC, which operates our New York clinic at 111 John St, Suite 570, New York, NY 10038, and Kalpraj Healthcare LLC, which operates our Jersey City clinic at 2815 JFK Blvd, Suite 207, Jersey City, NJ 07306. Together they operate the website at e-motionpt.com (the "Site"). "We" means the practice you deal with, and both practices for the Site. This Privacy Policy explains what information we collect through the Site and by phone and email, how we use it, who we share it with, and the choices you have. By using the Site you agree to this policy.

1. Information we collect

Information you give us. When you use a form on the Site or contact us directly, we collect:

• Appointment requests: first and last name, email address, phone number, preferred location, preferred dates and time of day, reason for visit, and your insurance carrier (optional).
• Contact form: first and last name, email address, phone number, preferred time to call, and your message.
• Careers: when you apply by email, the resume, cover note and contact details you choose to send.
• Calls and emails: the information you share when you call or email the practice.

Information collected automatically. Like most websites, our hosting provider records log data such as your IP address, browser type and version, the pages you visit, the time and date of your visit and how long you spend on pages, which is used to deliver, secure and improve the Site. The Site stores your light or dark mode choice and your selected location (New York or Jersey City) in your browser so the Site remembers them. A copy of a form you submit may also be kept in your own browser on your own device.

What we do not collect. We do not collect payment card numbers through the Site. We do not use advertising cookies, tracking pixels or third-party analytics on the Site. Please do not include detailed medical information, Social Security numbers or payment details in the free-text fields of our forms.

2. How we use your information

• To respond to your questions and requests, and to schedule, confirm and prepare for your visit
• To contact you by phone, text or email about your request or appointment
• To review job applications
• To operate, secure and improve the Site
• To keep business records and comply with legal obligations

We do not sell or rent your personal information, and we do not use it for advertising by third parties. We do not share your mobile phone number or text-messaging opt-in information with third parties or affiliates for marketing or promotional purposes.

3. Who we share it with

We share information only with service providers who help us run the Site and the practice, and only as needed to do so:

• Netlify hosts the Site and processes form submissions. Submissions are stored on Netlify's platform and emailed to our front desk.
• Google provides the maps shown on our location pages and the fonts used on the Site. Your browser connects to Google when these load, and Google may receive your IP address and browser information under its own privacy policy.
• Email and phone providers we use to communicate with you.

Service providers may access your personal information only to perform tasks for us, and they are required not to disclose it or use it for any other purpose.

We may also disclose information if required by law, to respond to legal process, or to protect the rights, safety and property of E-Motion, our patients or others.

4. Health information

Our Site forms are not a secure channel for health information and are not a substitute for speaking with your therapist. Health information you share with us as a patient during your care is handled under the federal and state health privacy laws that apply to our practice. We will explain how your health information is used and protected when you become a patient.

5. Calls, texts and email

When you submit a request, you agree that we may contact you by phone, text message or email to respond to it. Message and data rates may apply. To stop text messages, reply STOP to any message; to stop emails or calls, use the unsubscribe link or contact us. We will honor your request promptly. 6. Cookies and similar technologies

The Site does not use advertising or analytics cookies. It uses your browser's local storage only for the preferences described above. You can clear this at any time in your browser settings without affecting your ability to use the Site. If we add analytics in the future, we will update this policy first.

7. How long we keep information

We keep website requests and messages only as long as needed to respond, schedule your care and maintain our business records, and then delete them. Job applications are kept for as long as the role is open and a reasonable period afterward. Records we must keep by law are kept for the period the law requires.

8. Security

The Site is served over HTTPS, and form submissions are transmitted to our form processor over an encrypted connection. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.

9. Your choices and rights

You can ask us to tell you what personal information we hold about you, correct it, or delete it, and you can ask us to stop contacting you. Depending on where you live, you may have additional rights under state privacy laws, including the right to know, access, correct and delete your data and the right not to be treated differently for exercising those rights. To make a request, contact us using the details below. We will respond within 30 days and may need to verify your identity first.

10. Children

The Site is intended for adults. We provide care to minors with the involvement of a parent or guardian, and requests for a minor should be submitted by a parent or guardian. We do not knowingly collect personal information directly from children under 13 through the Site. If you believe a child has submitted information to us, contact us and we will delete it.

11. Third-party links

The Site links to other websites, such as Instagram and Google Maps. We are not responsible for their content or privacy practices and encourage you to read their policies.

12. Changes to this policy

We may update this policy from time to time. When we do, we will change the effective date at the top of this page. Your continued use of the Site after a change means you accept the updated policy.
